Community Based Health Care Service Design on Chronic Disease for Enhancing Primary Care and the Status of Community Based Primary Care Project

Abstract
The importance of chronic disease management in a rapidly aging society has emerged. One of limitations in existing government’s project for chronic disease management is lack of role for primary physicians. Since November 2014, the community based primary care project for strengthening primary care is being implemented in four regions. Chronic care model for community based primary care project focus on the educational counseling by clinic doctors. This projects include fee for service to doctors for education and counselling and referral activity to primary care support center. During 7 months, the education were conducted for 10,500 patients in 162 registered clinics. The result of survey for participated doctors and patients show that the patient satisfaction is very high and the counselling hours by doctors is prolongated and then the doctor patent rapport is improving. Therefore, this project is seen as ultimately contribute to enhancing primary health care.
