Risk Factors Associated with the Quality of Sleep of Patients with Chronic Obstructive Pulmonary Disease

Abstract
Background: Chronic obstructive pulmonary disease (COPD) frequently causes secondary insomnia and specific symptoms or physiological changes such as dyspnea and hypoxemia. Moreover, it is associated with several concomitant diseases. This study aimed to examine the factors associated with the quality of sleep of patients with COPD.
Methods: This cross-sectional study included 52 patients with COPD who were hospitalized between May 2015 and November 2015. By using the insomnia severity index (ISI), they were divided into two groups based on a score of 15 points. Patient characteristics such as age, sex, underlying disease status, body temperature, oxygen saturation, severity of pain, depression, anxiety, and hypnotic intake, as well as symptoms associated with sleep quality, in both groups were comparatively analyzed through logistic regression analysis.
Results: The mean ISI of all patients was 15.15 points. Of the 52 patients, 27 had ISI scores of 15 points or higher, thereby showing moderate-to-severe insomnia. In the logistic regression analysis, oxygen saturation of less than 95% showed an odds ratio of 4.325 (95% confidence 1.053-17.770), thereby indicating a significant and clear correlation with insomnia.
Conclusion: The quality of sleep of patients with COPD is closely related to a low oxygen saturation level. Hence, more studies should be conducted on the interaction between sleep quality and other factors such as hypercapnia.
