Behavioral and Psychological Symptoms in Male Patients with Early Stage of Alzheimer’s Disease and Vascular Dementia

Abstract
Background: Alzheimer’s disease and vascular dementia account for 80%-90% of all dementia. In addition to cognitive impairment, various behavioral and psychological symptoms appear that have significant clinical importance. Here, we investigated the prevalence and composite score of the neuropsychiatric symptoms in male patients with early stage Alzheimer’s disease and vascular dementia using Korean Neuropsychiatric Inventory (K-NPI).
Methods: Male patients diagnosed with Alzheimer’s disease or vascular dementia, who visited the dementia-psychiatry clinic of Gwangju Veterans Hospital from September 2014 to November 2015, were selected for the study. Out of these patients, only the ones with a clinical dementia rating score of 1 were included (Alzheimer’s disease 52 patients, vascular dementia 41 patients). We used K-NPI to examine the behavioral and psychological symptoms of dementia. The prevalence of these symptoms between the two groups was analyzed using chi-square test or Fisher’s exact test. ANCOVA was performed to analyze the composite score for each K-NPI domain between the two groups.
Results: Agitation/aggression, depression/dysphoria, disinhibition, irritability/lability were more prevalent in patients with vascular dementia than those with Alzheimer’s disease. Patients with vascular dementia had higher total K-NPI score and mean composite score in every domain except sleep/night time behavior than patients with Alzheimer’s disease.
Conclusion: Patients with vascular dementia had a higher prevalence of neuropsychiatric symptoms except sleep/night time behavior and mean composite score for most of the domains of K-NPI than patients with Alzheimer’s disease.
