Clinical Practice Guideline for Screening of Delirium in Hospitalized Older Adults

Abstract
Delirium is a complex neuropsychiatric syndrome commonly observed in the hospitalized older adults. The development of clinical practice guidelines for screening of delirium is important because delirium, which is a potentially preventable condition, often goes unrecognized among healthcare providers. We developed guidelines to assist physicians and nurses to optimize clinical outcomes and prevent delirium in the hospitalized older adults. We recommend the following: 1) Assess older adults for risk factors associated with delirium upon initial contact and if there is a change in an individual’s condition. 2) Assess older adults at risk for delirium daily (where appropriate) and whenever changes are observed or reported in an individual’s cognitive function, perception, physical function, or social behavior. 3) Use validated tools to assess older adults at risk for delirium. This guideline should be applied based on both, the needs of individual organizations or clinical practice settings and the needs and preferences of patients and their families accessing the healthcare system for care and services.
