Abstract
Type 2 diabetes is a metabolic disease of the heart that affects both micro and macro blood vessels. Cardiovascular disease is the leading cause of death in diabetic patients. Some diabetes medications reduce microvascular complications such as retinopathy, nephropathy, and neuropathy. However, they have not reduced macrovascular complications, such as myocardial infarction or stroke. Therefore, this article intends to specifically review the results of recent cardiovascular disease-related clinical trials for diabetes drugs. Metformin is the primary drug with benefits for cardiovascular disease. Sulfonylureas did not show cardiovascular benefits. Rosiglitazone, a thiazolidinedione class, increased the risk of heart failure, while pioglitazone improved cardiovascular outcomes. Among the DPP-4 inhibitors, saxagliptin increased hospitalizations for heart failure by 27%. Other DPP-4 inhibitors, except saxagliptin, had a similar incidence of major cardiovascular adverse events (MACE) to placebos. Recently, several clinical trials have been published on the safety of cardiovascular disease regarding SGLT2 inhibitors and GLP-1 receptor agonists in patients with cardiovascular disease. They have shown benefits not only in the safety of cardiovascular events, but also in the secondary prevention of cardiovascular diseases. SGLT2 inhibitors and GLP-1 receptor agonists reduced MACE in diabetic patients at risk for cardiovascular disease. Specifically, empagliflozin and liraglutide reduced cardiovascular deaths by 38% and 22%, and reduced all-cause deaths by 32% and 15%, respectively. Evidence suggests that GLP-1 receptor agonists and SGLT2 inhibitors, when used in combination therapy for type 2 diabetes mellitus, may provide additional benefits in lowering blood sugar as well as in preventing cardiovascular and renal events. GLP-1 receptor agonists reduced atherosclerotic ischemic events, and SGLT2 inhibitors mainly reduced heart failure and renal events. Accumulation of clinical data for both drugs could reduce the outcome of cardiovascular events in patients with diabetes and create new treatment options.

서 론

2형 당뇨병은 미세혈관(망막병증, 신병증, 신경병증) 및 거대혈관(심근경색, 뇌졸중) 모두에 영향을 미치는 심장대사 질환이다. 심혈관 질환(cardiovascular disease)은 당뇨병 환자의 주요 사망 원인이지만,1) 모든 당뇨병 환자에서 심혈관 질환 발병 위험이 동일한 것은 아니다. 당뇨병 환자에서 심혈관 질환에 대한 위험은 당뇨병 유병 기간에 따라 증가하고, 고혈압, 이상지질혈증, 대사증후군 및 만성콩팥병과 같은 다른 질환을 동반하고 있는지에 따라 영향을 받는다.2-4) 이미 기존에 심혈관 질환이 있는 당뇨병 환자는 후속 심혈관 질환 사건의 위험이 가장 높다.

당뇨병 치료에서 미세혈관 합병증(microvascular complications)과 대혈관 합병증(macrovascular complications)을 구별하는 것이 중요하다. 혈당을 강하시키는 전통적인 약물들(metformin, sulfonylurea 등)을 통해 혈당을 감소시키는 것이 다양한 작용 기전을 통해 망막병증, 신장병증, 신경병증과 같은 미세혈관 합병증을 줄이지만, 심근경색이나 뇌졸중 같은 대혈관 합병증을 감소시키지는 못했다.5-10) 심지어 일부 당뇨병 약제는 효과적으로 혈당을 강하시키지만, 역설적으로 오히려 심혈관 사건에 대한 부작용을 증가시키기도 했다.11,12) 티아졸리디네디온(thiazolidinedione) 계열의 로시글리타존(rosiglitazone)이 심근경색 및 심부전의 위험을 증가시킨다는 연구 결과로 인해, 당뇨병 약제의 심혈관 질환에 대한 위험성에 대한 경각심을 높였으며, 이 계열의 약제 사용이 임상에서 크게 감소했다.

이러한 당뇨병 치료 약제의 심혈관 질환 위험 증가에 대한 우려에 대응하기 위해, 2008년 U.S. Food and Drug Administration (FDA)은 모든 신규 당뇨병 약제에 대해 심혈관 질환에 대한 안전성을 입증하도록 의무화했다. 비교적 최근까지도 심혈관 질환이 있는 당뇨병 환자에 대한 대규모 전향적 무작위 임상 시험이 부족했지만, 2008년 FDA의 의무 규제를 통해 모든 당뇨병 신약은 적절하게 검증된 무작위 3상 임상시험에서, 허용할 수 없을 정도로 높은 심혈관 질환 사건 비율(주요 심혈관 사건에 대한 95% 신뢰 구간의 상한 구간이 1.8을 초과하지 않음)과 관련이 없음을 입증할 수 있는 경우에만 판매 허가를 받았다. 허가 후, 최소 2년 이상의 추적 관찰 동안 고위험 환자(기존 심혈관 질환 환자 포함)에 대한 심혈관 안전성(95% 신뢰 구간의 상한 구간이 1.3을 초과하지 않음)에 대한 4상 임상시험을 추가로 입증해야 했다.13)

이러한 FDA 명령의 결과로, 지난 몇 년 동안 기존에 심혈관 질환이 있는 환자를 대상으로 새로운 당뇨병 약제의 심혈관 질환 안전성에 대한 임상 시험이 꾸준히 발표되어 왔다. 대부분의 임상시험은 심혈관 사건에 대한 안전성을 입증하기 위해 설계되었지만, 일부 새로운 당뇨병 치료제는 심혈관 질환의 이차 예방측면에서 전례가 없는 이점을 보여주기도 했다. 본 종설에서는 최근 당뇨병 약제의 심혈관 질환 관련 임상시험 결과에 대해 구체적으로 검토하고자 한다.

당뇨병 약제와 심혈관 이익

1. Metformin (메트포르민)

메트포르민은 당뇨병 치료에 사용되는 유일한 비구아나이드 계열의 약제이며, 새로 진단된 당뇨병 치료를 위한 1차 약제로 주로 사용된다. 메트포르민은 인슐린 감수성을 증가시켜 혈당 수치를 낮추고 간 포도당 생성을 줄인다. United Kingdom Prospective Diabetes Study (UKPDS)에서 새로 진단된 과체중으로 구성된 2형 당뇨병 환자에서 10년간 추적 관찰을 했을 때, 메트포르민으로 치료받은 환자는 비치명적 심근경색 위험이 39% 감소했고(P=0.01), 모든 원인으로 인한 사망 위험도 36% 감소했다.7) 메트포르민은 심혈관 질환에 대한 혜택이 있는 1차 약제로써, 이 약제는 다른 약제로 대체하기 어려운 상태로 남아 있다. 최근 European Society of Cardiology/European Association for the Study of Diabetes (ESC/EASD) 가이드라인은 새로 진단된 심혈관 질환이 없는 2형 당뇨병(UKPDS 코호트)의 1차 예방에 대한 메트포르민의 유익한 효과를 인정하였다.14) 최근 심혈관 질환에 대한 이점을 증명한 몇몇 다른 종류의 당뇨병 약물과 비교했을 때, 메트포르민의 상대적인 이점 또는 위험도에 대한 연구는, 그에 필요한 임상 시험의 규모, 기간 및 비용을 고려할 때 이루어질 가능성은 거의 없다.

2. Sulfonylurea (설포닐유레아)와 thiazolidinedione (티아졸리디네디온)

설포닐유레아는 adenosine triphosphate (ATP)에 민감한 칼륨 채널(ATP sensitive potassium channel)을 결합하고 닫음으로써 인슐린 생산을 증가시킨다. 이것은 세포막의 탈분극으로 이어지며 차례로 칼슘 유입과 인슐린 분비의 자극을 유발한다. 당뇨병 치료에 사용되는 일반적인 약물 종류이며 심혈관계 이점을 나타내지 않는다.

티아졸리디네디온은 혈당 조절에 영향을 미치는 또 다른 종류의 약물이다. 이 약물은 감마 과산화소체 증식제-활성화제 수용체(PPAR-γ)를 활성화하여 골격근, 간 및 지방 조직에서 인슐린 감수성을 증가시켜 작용한다. 그러나 이 약물은 신장에서 나트륨 재흡수를 통해 체액 저류를 유발하는 것으로 알려져 있다. Rosiglitazone Evaluated for Cardiac Outcomes and Regulation of Glycemia in Diabetes (RECORD) 연구에서 로시글리타존은 심부전의 위험을 증가시키는 것으로 나타났다(위험비[hazard ratio, HR], 2.10; 95% confidence interval [CI], 1.35 to 3.27).15) 그러나, 피오글리타존(pioglitazone)은 혈당 강하 작용과 관계없이 죽상동맥경화 과정을 늦추거나 심지어 역전시켜 심혈관 결과를 개선시킨다.16-20) 이것은 티아졸리디네디온에 대한 클래스 효과가 아니며 로시글리타존에서는 이러한 효과를 입증하지 못했다.18)

그러나 American Diabetes Association (ADA)/EASD 합의문은 피오글리타존이 심혈관 질환에 대한 평가 변수를 감소시키는 것을 인정하기는 했지만, “이점에 대한 결정적인 증거는 없음(Without conclusive evidence for benefit)”이라고 기술하고 있다.21) 이러한 기술은 피오글리타존의 심혈관 이점에 대한 유일한 대규모 무작위 임상시험인 PROactive 연구의 1차 복합 평가 변수가 데이터 신뢰도에 대한 논란의 여지가 있기 때문인 것으로 보인다.22) 피오글리타존은 심부전 환자에게 금기이므로 사용 시 적절한 주의가 필요하지만, sodium-glucose cotransporter 2 inhibitor (SGLT2 억제제)를 같이 사용할 경우 피오글리타존과 관련된 체액 저류를 완화할 수 있다.23)

3. Dipeptidyl peptidase-4 inhibitor (DPP-4 억제제)

1) 작용기전

최근 인크레틴 생물학의 발전으로 새로운 종류의 경구용 항당뇨병 약물이 개발되었으며, 현재까지 glucagon-like peptide-1 (GLP-1)과 glucose-dependent insulinotropic polypeptide (GIP)와 같은 인크레틴 호르몬이 있으며,24) GLP-1은 경구 영양소 섭취에 대한 반응으로 장내 L-세포에 의해 분비되며, 포도당 의존적으로 인슐린의 분비를 자극하고 글루카곤(glucagon) 분비를 억제한다.24) 그러나 GLP-1과 GIP는 type II transmembrane glycoprotein인 DPP-4에 의해 빠르게 분해된다.24) 서로 다른 약리학적 특징을 가진 여러 DPP-4 억제제가 현재 이용 가능하며, 단독 요법으로 또는 다른 항당뇨병제와의 조합으로 사용할 수 있다. DPP-4 억제제의 기전은 GLP-1 및 GIP 같은 인크레틴 수준을 증가시켜 글루카곤 방출을 억제하여 인슐린 분비를 증가시키고 위 배출을 감소시키며 혈당 수준을 감소시키는 것이다.25,26) DPP-4 억제제는 당화혈색소 수준을 약 0.5%–0.8% 감소시키는 것으로 나타났으며, 임상 시험에서 DPP-4 억제제는 일반적으로 내약성이 우수하고 저혈당 위험이 낮았으며 체중이 증가하지 않았다.27)

2) 각 약제 별 임상연구 결과

(1) Sitagliptin (Trial Evaluating Cardiovascular Outcomes with Sitagliptin, TECOS study)

심혈관 질환을 동반한 2형 당뇨병 환자 14,671명에게 sitagliptin (시타글립틴)을 투여한 무작위 대조 연구인 TECOS 연구에서 평균 3.0년 관찰 시 위약군에 비해 시타글립틴 복용군에서 1차 주요 심혈관 질환(심혈관 사망, 비치명적 심근경색증, 비치명적 뇌졸중, 및 불안정형 협심증으로 인한 사망)이 열등하지 않았다(HR, 0.98; 95% CI, 0.88 to 1.09; P<0.001).28) 또한, 심부전으로 인한 입원율은 두 그룹 간에 차이가 없었다(HR, 1.00; 95% CI, 0.83 to 1.20; P=0.98).28)

(2) Saxagliptin (Saxagliptin Assessment of Vascular Outcomes Recorded in Patients with Diabetes Mellitus Thrombolysis in Myocardial Infarction 53, SAVER TIMI 53 study)

심혈관 질환의 과거력이 있거나 그 위험 요인들을 동반한 2형 당뇨병 환자 16,492명에게 saxagliptin (삭사글립틴)을 투여한 무작위 대조 연구인 SAVER TIMI 53 연구에서 평균 2.1년 관찰 시 1차 주요 심혈관 질환(심혈관사망, 비치명적 심근경색증, 비치명적 뇌졸중)은 삭사글립틴 복용군 613명에서 발생한 반면에, 위약군 609명에서 발생하였다(HR, 1.00; 95% CI, 0.89 to 1.12; P=0.99 for superiority and P< 0.001 for noninferiority). 한편, 위약군에 비해 삭사글립틴 복용군에서 심부전으로 인한 입원을 27% (HR, 1.27; 95% CI, 1.07 to 1.51; P= 0.007) 증가시켰다. 삭사글립틴을 사용한 DPP-4 억제는 심부전으로 인한 입원율은 증가했지만, 허혈성 질환들 발생 비율을 증가 또는 감소시키지 않았다. 삭사글립틴은 혈당 조절을 개선하지만, 당뇨 환자의 심혈관 위험을 줄이기 위해서는 다른 접근법이 필요하다.29)

(3) Linagliptin (Cardiovascular and Renal Microvascular Outcome Study with Linagliptin, CARMELINA study)

심혈관 및 신장 질환 위험이 높은 2형 당뇨병 환자 6,979명에게 linagliptin (리나글립틴)을 투여한 무작위 대조 연구인 CARMELINA 연구에서 평균 2.2년 관찰 시 리나글립틴 복용군에서 위약군에 비해 1차 주요 심혈관 질환 복합 결과(심혈관 사망, 비치명적 심근경색증, 비치명적 뇌졸중)의 발생률이 12.4% 대 12.1%로 나타났다(HR, 1.02; 95% CI, 0.89 to 1.17; P<0.001 for noninferiority). 리나글립틴은 위약과 비교하여 복합 심혈관 결과의 비열등한 위험을 보여주었다.30)

(4) Alogliptin (EXamination of CArdiovascular OutcoMes with AlogliptIN versus Standard of CarE, EXAMINE study)

급성관상동맥 증후군을 동반한 심혈관 질환 위험이 높은 2형 당뇨병 환자 5,380명에게 alogliptin (알로글립틴)을 투여한 무작위 대조 연구인 EXAMINE 연구에서 평균 18개월 관찰 시 알로글립틴 복용군에서 위약군에 비해 주요 심혈관 부작용(심혈관 사망, 비치명적 심근경색증, 비치명적 뇌졸중)의 발생률이 11.3% 대 11.8%로 나타났다(HR, 0.96; 단측반복 신뢰구간의 상한 1.16; P<0.001 for noninferiority).31) 즉, 위약군에 비해 알로글립틴 복용군에서 주요 심혈관 부작용의 발생률이 증가하지 않았다. 한편, 심부전으로 인한 입원의 재발 위험은 알로글립틴 복용군과 위약군에서 유사하게 나타났다(HR, 1.05; 95% CI, 0.82 to 1.34; P=0.707).32)

4. Sodium-Glucose Cotransporter 2 inhibitor (SGLT2 억제제)

1) 작용기전

정상 성인에서는 하루에 180 g의 포도당이 신장에서 여과되고, 근위세뇨관에서 모두 재흡수된다. 포도당 재흡수를 담당하는 SGLT에는 근위세뇨관 앞부분(S1 and S2 segment)에 위치한 SGLT2와 뒷부분(S2 and S3 segment)에 위치한 SGLT1이 있다.33) 정상 혈당에서 SGLT2는 여과된 포도당 재흡수의 97%를 담당하고, SGLT1은 3%를 담당하지만, 혈당이 너무 높거나 SGLT2가 억제되는 상황에서는 SGLT1에 의한 재흡수가 40%–50%까지 증가한다.34,35) 정상적으로 혈중 포도당이 200 mg/dL 이하에서는 여과된 포도당이 모두 재흡수되지만, 200 mg/dL를 초과하면 소변으로 포도당 배출이 시작된다. 혈당이 지속적으로 높은 당뇨병 환자의 경우, SGLT1과 SGLT2의 발현이 증가하고, 근위 세뇨관 세포 증식이 일어나서 여과된 포도당의 재흡수가 정상에 비해 증가한다.36) 신장에서의 포도당 재흡수는 근위 세뇨관의 Na+/K+-ATPase에 의해 세포 밖으로 Na+의 능동 수송에 의해 시작된다.37) Na+를 세포 밖으로 이동시켜 생성된 전기화학적 힘으로 SGLT를 통해 Na+와 포도당을 세포 내로 이동시킨다. 이때 포도당은 SGLT1의 경우, 2개의 Na+와, SGLT2의 경우, 1개의 Na+와 함께 이동하게 된다.38) 포도당이 세포 내로 이동하여 포도당 농도가 증가하면 세포와 간질 사이의 포도당 농도 차이에 따라 glucose transporter를 통해 포도당이 혈류로 다시 흡수된다.39) SGLT2 억제제는 신장 근위세뇨관에서 SGLT2를 억제하여 포도당 재흡수를 억제하여 혈당 감소 효과를 나타낸다. 이와 함께 체중 감소, 신장 기능 보호 및 혈압 감소 등의 효과도 나타낸다.40,41)

2) 각 약제 별 임상연구 결과

(1) Empagliflozin (Empagliflozin Cardiovascular Outcome Event Trial in Type 2 Diabetes Mellitus Patients-Removing Excess Glucose, EMPA-REG OUTCOME trial)

심혈관 위험이 높은 18세 이상 성인 2형 당뇨병 환자 7,020명에게 1일 1회 empagliflozin (엠파글리플로진) (10 mg 또는 25 mg)을 투여한 무작위 대조 연구인 EMPA-REG OUTCOME 연구에서 평균 3.1년 관찰 시 위약군에 비해 엠파글리플로진 복용군에서 주요 심혈관 질환(심혈관사망, 비치명적 심근경색증, 비치명적 뇌졸중) 발생이 14% 감소하였다(HR, 0.86; 95% CI, 0.74 to 0.99; P=0.04).42) 이외에 심혈관 질환으로 인한 사망이 38% (HR, 0.62; 95% CI, 0.49 to 0.77; P< 0.001), 심부전으로 인한 입원이 35% 유의하게 감소하였다(HR, 0.65; 95% CI 0.50 to 0.85; P=0.002).42)

이후 EMPA-REG OUTCOME 연구에 참여한 아시안 환자만을 따로 분석한 결과에서도 3-점 주요유해심혈관사건(3-point major cardiovascular adverse events, MACE)의 위험비(HR)는 0.68 (95% CI, 0.48 to 0.95)로, 모든 원인에 의한 사망이나 심부전에 대하여 서양인과 유사한 효과를 보였다.43)

(2) Canagliflozin (Canagliflozin Cardiovascular Assessment Study, CANVAS program/Canagliflozin and Renal Events in Diabetes with Established Nephropathy Clinical Evaluation, CREDENCE Trial)

심혈관 위험이 높은 2형 당뇨병 환자 10,142명에게 canagliflozin (카나글리플로진)을 투여한 무작위 대조 연구인 CANVAS program에서 평균 188.2주 관찰 시 위약군에 비해 카나글리플로진 복용군에서 심혈관 질환으로 인한 사망 및 심부전으로 인한 입원을 22% (HR, 0.78; 95% CI, 0.67 to 0.91)와 심부전으로 인한 입원 위험을 33% (HR, 0.67; 95% CI, 0.52 to 0.87) 감소시켰다.44) 참가자는 30세 이상으로 동맥경화성 심혈관 질환의 병력이 있거나 심혈관 질환의 위험인자(당뇨병 지속 기간 10년 이상, 항고혈압제 1개 이상을 복용 중이면서 수축기 혈압 140 mmHg 이상, 현재 흡연자, 미세단백뇨 또는 거대단백뇨, 고밀도 지단백 콜레스테롤 <1 mmol/L)가 2가지 이상인 50세 이상이라는 조건에 맞아야 하며, New York Association Class IV 심부전 환자는 제외되었다.44)

만성 신장 질환(eGFR이 30 to 90 mL/min/1.73 m2이며, urine albumin:creatinine ratio of >300 to 5,000 mg/g [>33.9–565.6 mg/mmol]인 알부민뇨 동반)을 가진 30세 이상의 2형 당뇨병 환자 4,401명에게 카나글리플로진을 투여한 무작위 대조 연구인 CREDENCE 연구에서는 평균 2.62년 관찰 시 위약군에 비해 카나글리플로진 복용군에서 주요 심혈관 질환(심혈관 사망, 비치명적 심근경색증, 비치명적 뇌졸중) 발생이 20% 감소하였다(HR, 0.80; 95% CI, 0.67 to 0.95; P= 0.01). 또한, 위약군에 비해 카나글리플로진 복용군에서 심혈관 질환으로 인한 사망, 비치명적 심근경색증, 비치명적 뇌졸중, 심부전으로 인한 입원 또는 불안정형 협심증으로 인한 입원을 26% (HR, 0.74; 95% CI, 0.63 to 0.86) 감소시켰다.45) 또한, 카나글리플로진 복용군에서 위약군에 비해 심부전으로 인한 입원이 39% 적게 발생하였다(HR, 0.61; 95% CI, 0.47 to 0.80; P<0.001).45)

(3) Dapagliflozin (Dapagliflozin Effect on Cardiovascular Events-Thrombolysis in Myocardial Infarction 58, Declare-TIMI 58 trial)

죽상동맥경화성 심혈관 질환 또는 그 위험요인들을 동반한 2형 당뇨병 환자 17,160명에게 dapagliflozin (다파글리플로진)을 투여한 무작위 대조 연구인 Declare-TIMI 58에서 평균 4.2년 관찰 시 위약군에 비해 다파글리플로진 복용군에서 심혈관 질환으로 인한 사망 또는 심부전으로 인한 입원을 17% (HR, 0.83; 95% CI, 0.73 to 0.95; P=0.005) 감소시켰다. 특히, 다파글리플로진 복용군에서 위약군에 비해 심부전으로 인한 입원이 27% 적게 발생하였다(HR, 0.73; 95% CI, 0.61 to 0.88).46)

5. Glucagon-like peptide-1 receptor agonist (GLP-1 수용체 작용제)

1) 작용기전

GLP-1은 음식 섭취 시 위장관 L-cell에서부터 생성되는 인크레틴 호르몬으로, 포도당 농도에 의존적으로 인슐린 분비를 촉진하고 글루카곤 분비를 감소시켜 혈당 강화 효과를 나타내며 위에서 음식물의 통과를 지연시키고 뇌에 작용하여 식욕을 억제해 복합적으로 혈당조절에 관여하며 체중 감소에도 도움이 된다.47) 또한, 췌장 베타세포의 기능을 호전시켜 인슐린 감수성에 좋은 영향을 준다.48) GLP-1 수용체 작용제는 작용 시간에 따라 분류하여 단기간 작용하는 약제로 exenatide, lixisenatide가 있고 지속작용하는 약제로 albiglutide, dulaglutide, liraglutide, semaglutide가 있다. 또, exendin-4를 기반으로 개발된 exenatide, lixisenatide와 GLP-1을 기반으로 합성된 albiglutide, dulaglutide, liraglutide, semaglutide로 나눌 수 있다.49) GLP-1 수용체 작용제는 체중 감소, 혈당 조절, 혈압 감소, 지질 개선 등 심혈관계 질환의 위험인자들을 개선시킨다.50) 또한 심장 및 내피세포에 직접적인 효과를 나타내는데, 산화질소 증가, endothelin-1의 감소, 활성 산소 감소, 염증 사이토카인 감소 등의 기전으로 죽상경화증의 진행을 예방하는 효과를 갖는 것으로 알려졌다.

2) 각 약제별 임상연구 결과

(1) Lixisenatide (Evaluation of Lixisenatide in Acute Coronary Syndrome, ELIXA)

ELIXA 연구는 180일 이내에 급성 관상동맥 증후군(acute coronary syndrome, myocardial infarction 또는 unstable angina)을 겪은 2형 당뇨병 고위험군 환자 6,068명을 대상으로 무작위로 lixisenatide군과 위약군으로 분류하여 평균 2.1년 관찰하였다. 1차 종료점은 심혈관사망, 비치명적 심근경색증, 비치명적 뇌졸중, 불안정 협심증으로 인한 입원으로 1차 종료점 발생률은 lixisenatide군에서 406명(13.4%), 위약군에서 399명(13.2%)으로 위약군 대비 위험도 비율(HR)은 1.02 (95% CI, 0.89 to 1.17)였다. 이는 위약에 대한 lixisenatide군의 비열등성을 나타내었지만(P<0.001) 우월성은 나타내지 않았다(P=0.81). 1차 평가변수의 개별 심혈관 성분과 관련하여, 각각의 빈도는 두 연구 그룹에서 유사했다. Lixisenatide군에서 총 156명의 환자와 위약군에서 158명의 환자가 심혈관 원인으로 사망했다(HR, 0.98; 95% CI, 0.78 to 1.22). Lixisenatide군에서 총 270명의 환자와 위약군에서 총 261명의 환자가 치명적 또는 비치명적 심근경색(HR, 1.03; 95% CI 0.87 to 1.22)이 있었고, lixisenatide군에서 총 67명의 환자와 위약군에서 60명의 환자가 치명적 또는 비치명적 뇌졸중(HR, 1.12; 95% CI, 0.79 to 1.58)이 있었고, lixisenatide군에서 총 11명의 환자와 위약군에서 총 10명의 환자가 불안정형 협심증으로 입원하였다(HR, 1.11; 95% CI, 0.47 to 2.62). 모든 원인으로 인한 사망은 lixisenatide군에서 211명(7.0%), 위약군에서 223명(7.4%)이 보고되었다(HR, 0.94; 95% CI, 0.78 to 1.13).51)

(2) Liraglutide (Liraglutide Effect and Action in Diabetes: Evaluation of cardiovascular outcome Results, LEADER)

LEADER 연구는 심혈관 위험인자를 동반한 심혈관 질환 고위험군 2형 당뇨병 환자 9,340명을 무작위로 liraglutide군과 위약군으로 분류하여 평균 3.8년 관찰하였다. 사건 발생 시간 분석의 1차 복합 결과는 심혈관 원인, 비치명적 심근경색 또는 비치명적 뇌졸중으로 인한 사망의 첫 번째 발생이었고, 1차 복합 결과 발생률은 liraglutide군에서 608명(13%), 위약군에서 694명(14.9%)이었다(HR, 0.87; 95% CI, 0.78 to 0.97; P<0.001 for noninferiority, P=0.01 for superiority). 심혈관계 원인으로 인한 사망은 위약군 278명(6.0%)보다 liraglutide군 219명(4.7%)에서 더 적은 수의 환자에게 발생했다(HR, 0.78; 95% CI, 0.66 to 0.93; P=0.007). 모든 원인으로 인한 사망률도 위약군 447명(9.6%)보다 liraglutide군 381명(8.2%)에서 낮았다(HR, 0.85; 95% CI, 0.74 to 0.97; P=0.02). 비치명적 심근경색증과 비치명적 뇌졸중의 빈도는 차이가 크지 않았지만 liraglutide군에서 위약군보다 낮았다.52)

(3) Semaglutide (Trial to Evaluate Cardiovascular and Other Long-term Outcomes with Semaglutide in Subjects with Type 2 Diabetes, SUSTAIN-6)

SUSTAIN-6 연구는 3,297명의 2형 당뇨병 환자를 대상으로 주 1회 제제인 semaglutide와 위약간 안전성을 비교하였다. 심혈관 질환(이전의 심혈관, 뇌혈관, 말초혈관 질환), 만성심부전 II–III, 3기 이상의 만성 신장 질환이 있는 50세 이상 환자이거나 하나 이상의 심혈관 위험 인자가 있는 60세 이상의 환자군이 포함됐다. 1차 복합 결과는 심혈관 원인, 비치명적 심근경색 또는 비치명적 뇌졸중으로 인한 사망의 첫 번째 발생이었고 1차 복합 결과는 semaglutide군에서 108명(6.6%), 위약군에서 146명(8.9%)이 발생하였다(HR, 0.74; 95% CI, 0.58 to 0.95; P<0.001 for noninferiority, P=0.02 for superiority). 비치명적 심근경색증은 semaglutide군에서 47명(2.9%), 위약군에서 64명(3.9%)이 발생했으며 유의미하지 않은 차이(HR, 0.74; 95% CI, 0.51 to 1.08; P=0.12)를 보였다. 비치명적 뇌졸종은 semaglutide군에서 27명(1.6%)과 위약군에서 44명(2.7%)이 발생했다(HR, 0.61; 95% CI, 0.38 to 0.99; P=0.04). 심혈관계 사망의 위험은 두 군에서 유사했으며 semaglutide군에서 44명(2.7%)와 위약군에서 46명(2.8%)의 사망이 보고되었다(HR, 0.98; 95% CI, 0.65 to 1.48; P=0.92).53)

(4) Exenatide (EXenatide Study of Cardiovascular Event Lowering; EXSCEL)

EXSCEL 연구는 이전에 심혈관 질환이 있었거나(70%), 심혈관 질환이 없는(30%) 2형 당뇨병 환자 14,752명을 중앙값 3.2년 동안 매주 1회 exenatide 또는 위약을 투여했다. 1차 결과는 사건 발생 시간 분석에서 심혈관 원인, 비치명적 심근경색 또는 비치명적 뇌졸중으로 인한 사망의 복합 결과의 모든 구성 요소가 처음으로 발생한 것으로 정의하였다. 2차 결과는 사건 발생 시간 분석에서 모든 원인으로 인한 사망, 심혈관 원인으로 인한 사망, 비치명적 또는 치명적 심근경색, 비치명적 또는 치명적 뇌졸중, 급성 관상동맥 증후군으로 인한 입원, 심부전으로 인한 입원을 포함하였다. 1차 복합 결과 사건은 exenatide군 839명(11.4%, 100인년당 3.7건)과 위약군 905명(12.2%, 100인년당 4.0건)에서 발생했다(HR, 0.91; 95% CI 0.83 to 1.00). 모든 원인으로 인한 사망 위험은 exenatide군에서 6.9%, 위약군에서 7.9%였다(HR, 0.86; 95% CI, 0.77 to 0.97). 이 차이는 통계적으로 유의한 것으로 간주되지 않았다. 첫 번째 치명적 또는 비치명적 심근경색, 치명적 또는 비치명적 뇌졸중의 비율 및 기타 이차 결과는 두 그룹 간에 유의한 차이가 없었다.54)

(5) Albiglutide (albiglutide and cardiovascular outcomes in patients with type 2 diabetes and cardiovascular disease, Harmony Outcomes)

Harmony Outcomes 연구는 2형 당뇨병 진단을 받고 관상동맥 질환, 뇌혈관 질환 또는 말초 동맥 순환 환자 9,463명을 대상으로 중앙값 1.6년 동안 가이드라인에서 권고하는 표준 치료와 함께 albiglutide군 또는 위약군에 무작위 분류됐다. 1차 결과는 치료 의향 인구에서 심혈관 원인으로 인한 사망, 심근경색 또는 뇌졸중으로 구성된 복합 결과의 모든 구성요소가 처음 발생한 것이었다. 1차 복합 평가 변수는 albiglutide군에서 100인년당 4.57 사건 발생률로 338명(7%)에서 발생했으며 위약군에서 100인년당 5.87 사건 발생률로 428명(9%)에서 발생했다(HR, 0.78; 95% CI, 0.68 to 0.90). 이는 albiglutide군이 심혈관 안전성에 대해 위약보다 열등하지 않고(P<0.0001 for non-inferiority) 효능 면에서 위약보다 우수함을 나타낸다(P=0.0006 for superiority). 복합 결과의 각 구성 요소에 대한 위험비는 심혈관 원인으로 인한 사망의 경우 0.93 (95% CI, 0.73 to 1.19), 심근경색의 경우 0.75 (95% CI, 0.61 to 0.90), 뇌졸중의 경우 0.86 (95% CI, 0.66 to 1.14)이었다. 모든 원인으로 인한 사망에 대한 위험 비율은 0.95 (95% CI, 0.79 to 1.16)였다.55)

(6) Dulaglutide (Researching cardiovascular Events with a Weekly INcretin in Diabetes; REWIND)

REWIND 연구는 이전에 심혈관 사건이나 심혈관 위험요인이 있었던 2형 당뇨병 환자 9,901명을 대상으로 dulaglutide군과 위약군으로 나누어 진행되었다. 연구 대상자 중 심혈관 질환자는 31%이고 심혈관 질환 병력이 없으며 위험요인을 동반한 환자 비율이 더 높았다. 1차 종료점은 비치명적 심근경색, 비치명적 뇌졸중, 심혈관 원인 또는 알려지지 않은 원인으로 인한 사망을 포함하는 복합 결과의 모든 구성 요소의 첫 번째 발생이었다. 중앙값 5.4년의 추적관찰 동안 치료에 따른 1차 복합 결과는 dulaglutide군 594명(12%, 100인년당 2.4명), 위약군 663명(13.4%, 100인년당 2.7명)에서 발생했다(HR, 0.88; 95% CI, 0.79 to 0.99; P=0.026). 1차 복합 결과의 세 가지 구성 요소 모두에 대해 일관된 효과가 관찰되었다. 심혈관 사망의 경우 위험비가 0.91 (95% CI, 0.78 to 1.06; P=0.21), 비치명적 심근경색의 경우 0.96 (95% CI, 0.79 to 1.15; P=0.65), 비치명적 뇌졸중의 경우 0.76 (95% CI, 0.62 to 0.94; P=0.017). 아울러 모든 원인에 의한 사망률은 dulaglutide군 10.8% (536명), 위약군 12% (592명)로 두 군 간 통계적으로 의미 있는 차이가 없었다(HR, 0.90; 95% CI, 0.80 to 1.01; P=0.067).56)

(7) Oral semaglutide (Peptide Innovation for Early Diabetes Treatment; PIONEER 6)

PIONEER 6 연구는 심혈관 혹은 만성 신장 질환을 동반한 2형 당뇨병 환자 3,183명을 대상으로 경구 semaglutide의 심혈관 안전성을 위약과 비교하였다. 중앙값 15.9개월 연구기간 동안 1차 결과는 무작위 배정부터 주요 심혈관 사건, 심혈관 원인으로 인한 사망(확인되지 않은 사망 원인 포함), 비치명적 심근경색 또는 비치명적 뇌졸중의 첫 번째 발생까지의 시간이 포함되었다. 1차 결과는 경구 semaglutide군에서 61명(3.8%), 위약군에서 76명(4.8%)이 발생했다. 위험도 21로 나타나 사전에 정의한 비열등성을 충족한 것으로 나타났다(HR, 0.79; 95% CI, 0.57 to 1.11; P<0.001 for noninferiority, P=0.17 for superiority). 1차 결과의 개별 구성 요소 중 심혈관 원인으로 인한 사망은 경구 semaglutide군에서 15명(0.9%), 위약군에서 30명(1.9%)에서 발생했다(HR, 0.49; 95% CI, 0.27 to 0.92). 비치명적 심근경색과 비치명적 뇌졸중 발생률에서는 유의미한 감소를 보이지 않았다. 다만, 모든 원인에 의한 사망은 경구용 semaglutide군에서 23명(1.4%), 위약군 45명(2.8%) (HR, 0.51; 95% CI, 0.31 to 0.84)에서 발생했다.57)

(8) Efpeglenatide (Effect of Efpeglenatide on Cardiovascular Outcomes; AMPLITUDE-O)

AMPLITUDE-O 연구는 2형 당뇨병과 심혈관 질환 병력 또는 현재 신장 질환이 있는 4,076명의 참가자를 대상으로 중앙값 1.8년 동안 진행하였다. Efpeglenatide군 189명(7.0%), 위약군 125명(9.2%)에서 적어도 하나 이상의 주요 심혈관계 질환이 발생하였고(HR, 0.73; 95% CI, 0.58 to 0.92), 신장질환 발생율은 32% (HR, 0.68; 95% CI, 0.57 to 0.79) 감소하였다.58)

결론적으로, 2형 당뇨병 환자에서 각 GLP-1 수용체 작용제의 심혈관질환에 대한 안전성을 위약과 비교했을 때, liraglutide, semaglutide, dulaglutide, albiblutide에서는 주요 심혈관계 사건(MACE)을 감소시킨 반면, lixisenatide, exenatide, oral semaglutide는 주요 심혈관계 사건의 감소 효과를 입증하지 못했다.59)

통합 분석에서 GLP-1 수용체 작용제로 치료하면 3P MACE에서 상대 위험도가 14% 감소했다(HR, 0.86; 95% CI, 0.80 to 0.93; P< 0.0001). NNT는 3.0년의 가중 평균 중앙값 추적 조사 동안 65 (45–130)였다. 복합 MACE 평가 변수의 구성요소를 별도로 평가할 때, GLP-1 수용체 작용제 사용은 심혈관 원인으로 인한 사망(HR, 0.87; 95% CI, 0.82 to 0.94; P=0.0010), 치명적 또는 비치명적 심근경색(HR, 0.90; 95% CI, 0.83 to 0.98; P=0.020) 및 치명적 또는 비치명적 뇌졸중(HR, 0.83; 95% CI, 0.76 to 0.92; P=0.0002)의 위험을 줄였다. 위약과 비교하여 GLP-1 수용체 작용제 치료는 모든 원인으로 인한 사망 위험이 12% 감소했고(HR, 0.88; 95% CI, 0.82 to 0.94; P=0.0001), NNT는 114 (76–228)였다. GLP-1 수용체 작용제로 치료받은 환자에서 심부전으로 인한 입원 위험도 11% 감소했고(HR, 0.89; 95% CI, 0.82 to 0.98; P=0.013), NNT는 258 (158–1,422)이었다(Table 1).60)

SGLT2 억제제와 GLP-1 수용체 작용제와의 병합요법

제2형 당뇨병에서 GLP-1 수용체 작용제와 SGLT2 억제제를 병용 요법으로 사용했을 때, 혈당 강하뿐만 아니라 심혈관 질환 및 신장 질환 예방에 추가적인 잠재적인 이득을 줄 수 있다는 증거들이 있다. 이 두 가지 서로 다른 클래스의 당뇨 약제는 여러 임상 시험에서 서로 다른 뚜렷한 작용 기전을 가지고 있고, 혈당 강하와 독립적이며 상호 보완적인 심혈관 사건 예방에 대한 이득이 있으므로,60-62) 이 두 가지 약제의 병용 요법이 단독 요법에 비해 더 포괄적인 유익한 결과를 나타낼 가능성이 높다. GLP-1 수용체작용제는 죽상경화성 허혈성 사건(atherosclerotic ischemic events, 주로 뇌졸중에 대한 이득이 더 크게 나타남)을 감소시키는 반면, SGLT2 억제제는 심부전, 신장기능 저하, 신장 질환을 주로 감소시키고 심근경색에는 중등도 정도의 감소를 보이며 뇌졸중에는 영향을 미치지 않았다.62) 실제로, 여러 가이드라인에서 동맥경화성 심혈관 질환이나 만성콩팥병의 위험성이 높은 당뇨병 환자의 경우 GLP-1 수용체 작용제를 먼저 사용한 이후 SGLT2 억제제를 사용하거나, 반대로 사용하는 것을 권장하고 있다.14,63-65)

그러나 이 두 가지 약제를 같이 사용하는 것에 대한 임상 시험 증거는 거의 없다. 본문에서 다루었던 임상 시험들에서 GLP-1 수용체 작용제의 임상에서 연구 시작 시점에 SGLT2 억제제를 같이 사용한 경우는 불과 0%–5.3% 사이였으며, SGLT2 억제제의 임상에서 연구시작 시점에 GLP-1 수용체 작용제를 같이 사용한 경우는 불과 2.5% 에서 4.4%였다. 이러한 데이터를 바탕으로 시행한 체계적인 문헌 고찰 및 메타분석에서 GLP-1 수용체 작용제와 SGLT2 억제제를 같이 사용한 1,913명을 분석했으나, 대부분 당화혈색소의 변화, 체중 변화에 대한 6개월 정도의 단기 보고만 있었고, 심근경색, 뇌졸중 사건에 대해 보고한 연구는 없었다. 그러나 이 메타분석 연구에서 두 가지 약제를 같이 사용한다고 해서 저혈당의 위험성을 추가로 증가시키지는 않았다는 점을 보여주었다.66)

결 론

SGLT2 억제제와 GLP-1 수용체 작용제는 심혈관 질환 위험이 있는 당뇨병 환자에서 주요 심혈관계 사건(MACE)을 줄였지만, 이러한 계열의 모든 제제가 심혈관 질환으로 인한 사망 및 모든 원인으로 인한 사망을 감소시킨 것은 아니었다. SGLT2 억제제인 엠파글리플로진과 GLP-1 수용체 작용제인 리라글루타이드만 심혈관 질환으로 인한 사망을 각각 38%, 22% 줄이고, 모든 원인으로 인한 사망을 각각 32%, 15% 줄였다.

SGLT2 억제제의 심혈관 이점은 1–2개월 이내에 관찰될 수 있는 반면, GLP-1 수용체 작용제의 심혈관 이점을 보려면 1.5–2년이 필요하며, 이는 심혈관계에 대한 이 두 약제의 서로 다른 작용 기전과 일치한다. 따라서, 당뇨병 환자를 치료함에 있어서 이중 또는 삼중 요법이 실제로 단일 요법에 비해 추가적인 이점을 제공하는지 확인하기 위해 피오글리타존, SGLT2 억제제 및 지속형 GLP-1 수용체 작용제의 조합에 대한 시험이 이루어져야 할 충분한 증거가 쌓이고 있다. 이를 통해 당뇨병 환자의 심혈관 사건 결과를 줄이고 생명을 구하는 새로운 치료 옵션이 생길 수 있다.
