Physicians’ Experience from the Pilot Project for Primary Care for People with Disabilities

Abstract
Background: Disabled people are considered a vulnerable population in terms of health and social care. With the disability act having been enacted in 2015, the primary care physician system for disabled people was designed and implemented as a pilot project as of May 2018. This study was designed to assess the suitability and effectiveness of this pilot project. To this end, we conducted a survey of medical staff who participated in the pilot project, making use of questionnaires derived from preceding focused group interviews.
Methods: We first conducted focused group interviews with experts who were actively engaged in the pilot project. Questionnaires were then prepared based on these focused group interviews, which were then administered electronically to 23 physicians who had provided medical services for disabled people throughout the pilot project.
Results: In this study, physicians noted that all the services in the pilot program are useful for the healthcare of disabled people in the following order: comprehensive evaluation, education and counselling, coordination of care and medical referral, and comprehensive planning. The biggest problem identified was the complexity of the administrative procedure and the inadequacy of medical fees. The participants endorsed greater participation of secondary/tertiary hospitals, the establishment of links within the local community, and the strengthening of the role of public medical centers.
Conclusion: All the services in the pilot program appear to be useful in the provision of healthcare of disabled people, and several measures are suggested for the improvement of the program prior to the actual implementation of the project.

서 론

장애인은 의료에 대한 접근성1) 및 건강 관리에 있어서 많은 제한점을 가지고 있는 집단2)으로 고령화와 더불어 다양한 장애발생 위험요인의 증가에 의해 장애인구는 지속적으로 증가하는 추세이다. 장애인은 일반 인구집단과 비교하여 만성질환 및 중증질환 이환율이 높게 보고 되었으며3,4) 의료진은 장애 감수성 및 장애인을 진료하기 위한 지식과 기술 등이 부족한 상황이다. 이에 2015년 장애인 건강권 및 의료 접근성 보장에 관한 법률이 제정되었으며, 2018년 5월부터 장애인 건강 주치의 제도 시범사업이 시행되었다. 본 연구에서는 의료진의 관점에서 시범사업의 문제점을 파악하고 개선방안을 도출해 보고자 한다.

방 법

2018년 10월부터 2019년 3월까지 시범사업에 참여한 서울, 수도권, 충청 지역 주치의들을 대상으로 초점 집단면접을 시행하였고, 이를 바탕으로 만족도 조사 설문항목이 개발되었다. 만족도 설문 항목은 사업의 추진 체계, 건강관리서비스 제공의 프로세스, 정보시스템의 사용, 만족도, 불만 사항, 장애요인, 개선점 등을 포함하였으며 시범사업에 등록한 의사 23명을 대상으로 2019년 4월부터 6월까지 실시되었다. 본 연구는 삼성서울병원 임상연구윤리위원회(Institutional Review Board)의 승인을 받아 시행되었다(IRB 승인번호: 2019-07-008).

결 과

1. 질적 연구

포괄 평가를 통해 충분한 시간을 할애할 수 있게 된 점, 방문 진료를 통해 접근성이 어려운 장애인들의 진료가 가능했다는 긍정적인 면이 있었지만 여러 문제점들이 제기되었다. 건강보험공단과 심평원의 중복 등록 및 청구가 분리되어 있는 등 절차의 복잡성, 1–3급 장애인의 경우 이미 관리를 받고 있는 병원이 있어 새로운 주치의 제도의 필요성을 느끼지 못한다는 문제점이 있었다. 포괄 평가 항목은 필수 입력 항목 및 의사 혼자 평가하기 어려운 항목들이 많음을 지적하였고, 방문진료 시 환자는 약 처방, 소변줄 교체, 욕창 치료 등의 치료를 주로 원하지만 이에 대한 진료행위 및 청구를 할 수 없다는 점이 제도상의 문제로 제기되었다. 또한 의료급여환자 이외의 환자들은 주치의 등록 시 첫 진료부터 본인부담금이 발생하기 때문에 큰 장애요인이 된다는 공통된 의견이 있었다.

2. 만족도 조사

시범사업에 등록한 의사 중 서비스를 1회 이상 제공한 경험이 있는 의사 30명을 대상으로 온라인 설문 조사를 실시하여 23명이 응답하였다. 남자는 17명, 여자 6명이었으며, 연령대는 50대가 47.8%로 가장 많았다. 진료과는 가정의학과가 39.1%로 가장 많았으며, 시범사업에 참여한 경위는 복지부 및 공단의 공식요청(43.5%)을 통해서 가장 많이 참여하였다. 포괄평가는 1명을 제외하고 환자의 상태 파악에 도움이 된다고 응답하였고, 문제점으로는 초진에서 포괄평가에 요구되는 항목을 모두 파악하기 어려움이 가장 높게 나타났다. 방문진료는 시행하지 않은 경우가 52.2%로 이유는 시간/인력이 부족해서 72.7%, 요청환자가 없어서 45.5% 순으로 응답하였다. 방문서비스에서 장애인들이 필요로 한다고 생각하는 서비스는 모두가 전화상담이라고 응답하였고, 모든 방문간호 행위 서비스는 82%–95.7%까지 필요하다고 생각하는 것으로 나타났다. 방문서비스 개선을 위하여 필요한 것은 환자 본인부담금 경감(82.6%), 수가현실화(73.9%) 순으로 응답하였다.

시범사업의 서비스들은 모두 장애인 건강관리에 도움이 되는 것으로 응답하였으며, 포괄평가, 교육상담, 진료의뢰, 연계, 종합계획 수립 순으로 나타났다(Table 1). 시범사업에 대한 만족 수준은 ‘전혀 만족하지 않는다’를 포함하여 만족하지 않는 쪽이 56.6%로 나타났으며 그 이유는 행정절차의 복잡성과 수가의 부적절성이 가장 높았다. 행정절차에 있어 장애인 등록과 서비스 내용 등록 절차에 대한 불만족이 73.9%로 매우 높게 나타났다. 시범사업 개선안에 대해서는 상급의료기관 참여 확대, 지역사회 협의체 마련, 보건소, 공공의료원 역할 확대 순으로 응답하였다(Table 2). 본인부담금 관련해서는 본인부담금을 낮추는 것에 대해 100% 찬성 응답하였으며 비율은 평균 5.2%로 낮추는 의견을 가장 많이 응답하였다.

고 찰

장애인은 다수가 복합적인 건강 필요를 가지고 있음에도 불구하고 의료진은 장애 자체에만 초점을 맞추기 쉽고, 이에 따라 만성질환 관리나 건강습관에 대한 관리가 잘 이루어지고 있지 않은 실정이다. 그러나 장애인이 장애 관련 질병을 보는 의사 이외에 일차 진료의사를 함께 보는 경우에는 필요한 검진을 수행하는 확률이 훨씬 높은 것으로 나타났으며,5) 이는 장애인이 주치의를 두는 것이 장애인의 건강관리와 결과 향상에 중요하게 작용할 것으로 예상해 볼 수 있다. 본 연구를 통해서도 전문가 집단은 시범사업의 서비스들은 모두 장애인 건강관리에 도움이 되는 것으로 평가하였다.

시범사업의 가장 큰 문제점으로는 행정절차의 복잡성과 수가의 부적절성으로 국내에서 수가에 대한 논의는 가장 현실적인 사안으로 얼마의 수가를 책정해야 의사의 참여를 이끌어 낼 수 있을지 논의가 필요하며, 공급자의 의견뿐만 아니라 수요자인 환자의 의향을 고려하여 책정되어야 하겠다. 환자 등록 권유가 어려웠던 가장 큰 이유로 환자본인부담금 증가로 인해 환자 부담이 커졌기 때문이라는 점을 고려하였을 때 본인부담금 경감에 대한 대책도 필요할 것으로 보인다. 시범사업에 참여한 의사들은 보건소 건강생활 지원센터와 함께 장애인을 관리할 의향이 78.3% 있는 것으로 나타나 향후 지역 장애인 보건의료센터, 보건소, 지역의 의료서비스 및 장애인 지원 인프라 등 여러 기관과의 효과적이고 효율적인 연계를 위한 방안이 모색되어야 할 것으로 생각된다.
