Disabled People’s Experience and Perception on the Pilot Project for Primary Care for People with Disabilities

Abstract
Background: Compared with the general population, people with disabilities have a higher rate of morbidity and mortality; however, their access to health and medical care remains limited. In May 2018, a pilot project of primary care for people with disabilities was implemented.
Methods: We conducted qualitative research by interviewing people with disabilities; 101 persons with disabilities were surveyed so as to quantitatively gauge their satisfaction and to examine their experiences and perceptions of the pilot project.
Results: Persons with disabilities were largely aware of medical accessibility problems caused by difficulties in moving, access to facilities, and access to equipment. Satisfaction with home visitation care and nurse visitations was very high. The participants had various unmet needs – such as the need for physical therapy or drug delivery services.
Conclusion: This study identified problems faced by people with disabilities and suggests that more effective and realistic solutions are needed to meet their needs before implementing the original project.

서 론

장애인은 일반 인구집단과 비교하여 만성질환 및 중증질환 이환율이 높고,1,2) 건강 및 의료 접근성에 대해 많은 제한점을 갖고 있다. 장애인은 편리한 시간에 외래를 방문하기 어렵고, 시설, 장비 등 구조적인 문제가 있으며, 의료기관을 방문하기 위한 교통수단도 제한적이어서 접근성 문제가 비장애인에 비하여 현저히 높다.3)

따라서 장애인이 주치의를 두는 것은 장애인의 의료 접근성 및 건강향상에 중요하다. 이런 배경으로, 장애인 건강증진 및 의료 접근성 보장을 위한 장애인 건강 주치의 시범사업이 2018년 5월부터 시행되었다. 본 연구에서는 장애인의 관점에서 현행 시범사업에 대한 문제점을 확인하고 개선방안을 도출해보고자 하였다.

방 법

1. 질적 조사

사업에 참여한 장애인 15명 및 보호자 2명을 대상으로 2018년 11월–12월에 걸쳐 8회의 인터뷰를 시행하였다.

2. 양적 설문 조사

총 101명을 대상으로 설문 조사를 실시하였고, 현행의 장애인 건강 주치의 시범사업에 대한 문제점을 확인하였다. 본 연구는 삼성서울병원 임상연구윤리위원회(Institutional Review Board)의 승인을 받아 시행되었다(IRB 승인번호: 2019-07-008).

결 과

1. 질적 조사

계속 진료받던 의사에게 주치의 등록을 권유받아 참여한 경우가 많았다. 이동의 어려움으로 가정방문 필요성이 높다는 의견이 있었고, 간단한 방문 간호 처치뿐만 아니라 근육 강직 등에 대한 방문 물리치료, 우울증에 대한 심리치료사 등 다양한 방면의 가정 방문을 원하는 의견이 있었다. 의원은 약국을 따로 가야 하는 것이 문제점으로 제시되었으며 약 배달이 되었으면 좋겠다는 의견이 있었다. 의료급여환자 이외에는 주치의 등록 후 지불하지 않던 새로운 진료비를 내야 한다는 점을 지적했고 장애인 건강 주치의 병원에 가서 본인 부담금이 경감된다면 그 병원을 주로 가게 될 것 같다는 의견이 있었다. 의사들이 적극적으로 참여할 수 있도록 수가가 충분히 보상되어야 한다는 의견도 있었다. 의사들의 장애인에 대한 인식 개선, 장애 이해 교육이 필요하다는 의견이 있었다.

2. 양적 설문 조사

참여자는 지체장애가 57명(56.4%)으로 가장 많았고 그 다음으로는 뇌병변 장애 24명(23.8%)이었다. 장애 등급별로는 1급 45명(44.6%), 3급 31명(30.7%), 2급 25명(24.8%) 순서였다. 참여 동기는 의료인의 권유가 58.4%, 주변 사람의 소개 24.8%, 주변 단체나 다른 기관 소개 약 9.9% 순이었다.

장애인용 주차장 34.7%, 장애인용 화장실 18.8%, 이동(문턱, 엘리베이터 등) 14.9%, 진료(검사, 처치, 의료장비 등) 8.9%, 의사소통 5.0% 순으로 주치의 등록기관이 불편하다고 하였다. 시범사업 참여 후 평균 진료시간이 15.8분으로 참여 전 13.1분보다 2.7분 증가하였고, 10분, 15분, 20분, 30분 진료시간이 참여 전보다 참여 후가 증가하였다. 참여자의 90.1%는 주치의에게 궁금한 점이나 걱정스러움을 말할 충분한 기회가 있었다고 대답하였다. 주치의가 관리하는 항목은 주로 건강관련 문제 지속관리 및 일상적 건강문제로 나타났고, 제공받은 서비스는 전화상담, 포괄평가 및 종합계획, 방문 진료, 교육 상담, 방문 간호 순서였다. 전반적으로 만족하는 것으로 응답하였으며, 특히 방문 진료와 방문 간호는 매우 만족이 81% 이상으로 매우 높았다(Table 1). 시범 사업 후 생긴 변화로는 진료에 대한 이해도 상승 78.2%, 의사에 대한 신뢰감 상승 77.2%로 응답하였고, 장애에 대한 이해도 상승, 진료 만족도 상승, 환자 의견 치료에 반영 증가, 정기적 병∙의원 방문 증가, 장애관리와 생활습관관리에 대한 중요성 인식 등 전반적으로 긍정적인 변화가 있었다고 응답하였다. 약 97%의 참가자가 향후 건강 주치의 사업에 지속 참여 의향이 있다고 밝혔다.

급한 증상 등에 대한 전화상담, 장애/건강관리에 대한 다양한 교육제공, 주기적 장애/건강관리 제공이 각각 94.1%, 93.1%, 92.1%로 가장 높게 나타났으며, 의료진의 방문 모니터링, 방문 수액치료, 방문진료, 방문채혈, 방문물리치료, 사회복지사 방문상담제공 요구도도 높게 나타났다. 그 외 약사방문 복약상담 및 약 배달 요구도도 높았다(Table 2).

고 찰

본 연구에서는 질적, 양적 조사를 통해 장애인 건강 주치의 시범사업에 참여한 장애인들의 다양한 경험과 개선에 대한 의견을 확인할 수 있었다.

장애인 건강 주치의 등록에 있어 의료인의 권유가 가장 중요한 것으로 나타났는데, 의료인의 적극적이고 자발적인 참여를 도모하기 위하여 수가가 충분히 보상되어야 할 것이다. 또한 주치의 등록 후 새로이 발생한 본인 부담금에 대한 문제점을 지적한 의견이 있었던바, 본인 부담금 할인 등의 제도적 방법에 대한 논의도 함께 필요할 것으로 보인다. 시범 사업 후 실제적인 진료의 양과 질 개선이 이루어진 것으로 생각되므로 이는 포괄 평가 및 계획수립료, 교육 상담료 등의 수가 산정이 필요한 타당한 이유가 될 것이다.

제공된 서비스 중에서는 방문 진료와 방문 간호에 대한 만족도는 매우 높았다. 방문 서비스는 장애인 건강 주치의 시범 사업의 특징 및 의의이고 방문수가가 제공되는 것에 대한 타당한 근거가 될 것이다. 그러나 방문 진료 및 방문 간호에 대한 장애인의 높은 만족도에 비해, 실제 시범 사업 참여시 방문 진료와 방문 간호를 받은 경우는 각각 26.7%, 20.8%로 나타나 추후 방문 서비스 활성화 및 확대화가 중요할 것으로 판단된다.

필요한 서비스로는 급한 증상 등에 대한 전화상담, 장애/건강관리에 대한 다양한 교육제공, 주기적 장애/건강관리 제공이 높게 나타나, 장애인 진료 역시 지속적이고 포괄적 의료가 제공되어야 함을 알 수 있다. 또한 의사, 간호사의 방문뿐만 아니라, 방문 물리치료, 사회복지사의 방문 상담, 약사의 방문 복약 상담 및 약 배달에 대한 요구도도 높게 나타났다. 이는 방문 서비스가 조금 더 포괄적이고 다양하게 제공되어야 함을 보여주며 이를 통해 장애인의 의료 접근성 향상을 기대할 수 있을 것으로 생각한다. 향후 충분한 인력 확보 및 정책 혹은 제도의 뒷받침이 필요할 것이다.

본 사업만으로는 시범사업 참여가 장애인의 건강 관리 능력 향상에 실질적으로 어떤 도움이 되는지를 입증하기에는 한계가 있어 추후 이에 대한 실증 연구가 추가로 필요할 것으로 생각된다.

본 사업을 시행함에 앞서 장애인이 현실적으로 직면하고 있는 문제점을 확인하고 만족도가 높은 서비스를 찾아내어 그들의 요구에 맞는 보다 효과적이고 현실적인 해결책을 찾고자 함에 본 연구의 의의가 있겠다.
